Meet Landrey Fagan of Four Mile Creek Health

Today we’d like to introduce you to Landrey Fagan

Hi Landrey, it’s an honor to have you on the platform. Thanks for taking the time to share your story with us – to start maybe you can share some of your backstory with our readers?
I’m a family physician with over 15 years of experience and a deep passion for preventive medicine, menopause care, and longevity. My journey into medicine began with a fascination for how hormones shape our physiology. During graduate school, in the wake of the Women’s Health Initiative (WHI) trial cancellation, I wrote my master’s thesis as a counterpoint to the claims and findings of the WHI, advocating instead for bio-identical transdermal hormone replacement therapy. That early work solidified my commitment to understanding and addressing the unique challenges women face across their lifespans.

After completing my medical training, I chose family medicine to provide comprehensive care for both women and men. Over the years, I’ve focused my practice on metabolic health, preventive care, and hormone management, earning board certification in preventive medicine and becoming certified as a menopause provider through the North American Menopause Society (NAMS). Over the course of my career, I’ve seen how insurance can stand in the way of ideal preventative medicine. I have also found these nuanced conversations about menopause and health optimization cannot be contained in a 15 minute visit model. Because of these limitations in the “mainstream” medical system, I started a direct primary care practice that allows me to provide the most attentive care to my patients.

Today, I’m passionate about helping my patients navigate the complexities of the healthcare system, optimizing health in many ways including through menopause, tailoring care, family history considerations, and advanced screening tools. My goal is to empower people to take charge of their health, optimize their well-being, and live long, vibrant lives.

Alright, so let’s dig a little deeper into the story – has it been an easy path overall and if not, what were the challenges you’ve had to overcome?
Writing my master’s thesis on bio-identical transdermal hormone replacement therapy wasn’t just an academic exercise—it was a way to address the widespread fear and misunderstanding about hormone therapy at the time. It was daunting to push against the prevailing beliefs at the time, but it reinforced my commitment to advocating for evidence-based care. I am grateful that the truth about the safety of hormone replacement therapy is finally getting the attention it deserves.

After completing medical training, I chose family medicine for its breadth and opportunity to provide whole-person care. Yet, balancing the demands of my early career with my passion for women’s health and my family was not easy. Like many in medicine, I faced burnout—feeling stretched thin while trying to provide the kind of care I believed every patient deserved. I struggle still with enforcing boundaries and protecting family time.

Personal challenges also shaped my understanding of resilience and health. My youngest daughter has struggled with epilepsy. After years of sleepless nights as parents we ultimately discovered she has a rare genetic mutation which stunned the family. Among many treatments we tried, she required a strict ketogenic diet for several years. As a result we have all learned so much about nutrition and the challenges of being a patient in our medical system. I now more deeply appreciate the need to incorporate nutrition, supplements and other alternative therapies to meet individual needs often alongside mainstream medical care.

Starting my own practice has been another test. I have left the familiarity of working in established clinics over 15 years to build something from the ground up. Over the past several months I’ve been stepping out of my comfort zone, navigating the complexities of entrepreneurship, and overcoming self-doubt. Yet, it is providing me the chance to create a space where I can focus on what I truly value: empowering patients to thrive.

I hope that my personal and professional struggles have made me a better physician and a more compassionate human being.

At Four Mile Creek Health, I’ve created a direct primary care clinic that reflects my commitment to providing personalized, comprehensive healthcare. With over 15 years of experience as a board-certified family physician, I’m passionate about focusing on prevention, metabolic health, and menopause care.

What sets my clinic apart is the time I take with each patient. I believe in longer visits that allow for meaningful conversations, tailored treatment plans, and advanced tools like continuous glucose monitors and SECA body composition analysis. From nuanced hormone management to advanced cancer screening, my goal is to empower patients with the knowledge and care they need to prosper.

Can you talk to us a bit about happiness and what makes you happy?
My children are a central source of joy. Watching them grow, pursue their interests, and achieve milestones makes my heart full.
Having the privilege of providing medical care and being a trusted confidant for my patients and an advocate for them is deeply fulfilling. I love to continue to learn and grow in medicine and I’m excited about new developments in healthcare and being able to share these new therapies with others.
